Police Department Firearms
ALL FIREARMS APPLICANTS - PLEASE READ CAREFULLY!!
  1. All applications submitted should be both handwritten neatly and legible, manually typed or filled out utilizing the PDF forms provided on the New Jersey State Police website or located below.

  2. All addresses must be complete with house number, street, town, state, and zip code.

  3. Height shall be in feet and inches (example 6'3")

  4. Indicate race with either Asian, Black, American Indian or White.

  5. All forms must be signed. It is requested you complete this in the presence of the Berlin Police Department employee or law enforcement officer when you submit the application.

  6. If you are applying for a Handgun Purchase Permit, there is no limit on the quantity of permits you may apply for.

  7. You must appear in person with your application with two forms of identification. One form must be a valid Government issued form of identification (i.e. NJ driver’s license, Passport).

  8. All notifications are made by mail. Please DO NOT call The Berlin Police Department as the processing of your application may take some time to process.

The following reasons will be just cause for denial of your firearms application regardless if you have a previously approved application on file:

  • Incomplete Application – This includes but is not limited to incomplete name, address and telephone numbers for references and/or employer.

  • Any block left blank. If it does not pertain to you, please list N/A or None. Do not leave it blank.

  • Applications must be legible. ILLEGIBLE handwriting will not be accepted.

  • If any page of your application is not considered acceptable, your application will be denied. You will not be called to make corrections.

  • If you are denied you will receive a letter and have the right to appeal the decision with the Camden County Superior Court (instructions for appeals will be provided on your letter of denial).

* The falsification of information on any of the applications for firearms permits is a violation of N.J.S. 2C:39-10C and is crime of the third degree. Any falsification may result in criminal charges against you.

FIRST TIME APPLICANTS for a Firearms Purchaser Identification Card and/or Handgun Purchase Permit

  1. All first time applicants MUST be fingerprinted. The fingerprint process will be completed by MorphoTrust. You must obtain the form from the Berlin Police Department PRIOR to scheduling your appointment.

  2. Schedule an appointment with MorphoTrust by going to www.bioapplicant.com/nj or by calling 877-503-5981.

  3. Complete the Consent For Mental Health Records Search (Form SP 66). It is requested you sign this in the presence of the Berlin Police Department employee or law enforcement officer when you submit the application.

  4. Complete a State of New Jersey Application for Firearms Purchaser Identification Card and/or Handgun Purchase Permit (Form STS-033).

  5. References should not be relatives (this includes fiancé, girlfriend, boyfriend, etc.). Telephone numbers, addresses and zip codes must be complete.

SUBSEQUENT APPLICANTS for Additional Permits to Purchase a Handgun and/or Duplicate Firearms Purchaser Cards

  1. All applications for additional purchase permit and/or for a Firearms Purchaser Identification duplicate card (i.e. mutilated, lost or stolen, change of address, name and/or sex) must fill out the State of New Jersey Application for Firearms Purchaser Identification Card and/or Handgun Purchase Permit (Form STS-033).

  2. Complete the Consent For Mental Health Records Search (Form SP 66). It is requested you sign this in the presence of the Berlin Police Department employee or law enforcement officer when you submit the application.

  3. All subsequent applicants are required to complete the State Bureau of Identification (SBI) Form 212A as a Criminal History Records Check must be conducted. (NOTE: Fingerprinting may be required and is at the discretion of the Chief of Police.)

Application for a Permit to Carry a Handgun
(The following instructions are the same for the initial and renewal application)

Application to State
:

  • Live out of state

  • Armored car employee

Application to Chief of Police of Municipality

  • Complete a State of New Jersey Application For Permit To Carry A Handgun, Form S.P. 642 in triplicate. All references must know the applicant for a minimum of three (3) years prior to the date of the application.


  • All original copies must be notarized.

  • Submit four (1.5” x 1.5”) color passport size photographs with your application package.

  • Complete the Consent For Mental Health Search, Form SP-66.

  • Must be fingerprinted with each application every two (2) years.

  • Submit in writing a justifiable reason / need for the issuance of a permit to carry a handgun. This must be detailed. Armored car guards shall obtain this from the C.E.O. of the company they are employed by. This shall also be notarized.

  • Written proof of qualification with the handgun(s) you intend on carrying if your application is approved. This must be recent at the time of the application and must also be obtained from a certified firearms instructor.

  • A money order in the amount of $50.00 payable to, “Treasurer - State of New Jersey.” (Amount is subject to change.)

  • All armored car guard applications shall be submitted to the appropriate New Jersey State Police Barracks. All others shall be submitted to the law enforcement agency where the applicant resides. If your town of residence is covered by a State Police barracks on a full time basis, submit to that barracks. If part time, submit to that municipal police department. All out of state applicants must submit to the closest New Jersey State Police Barracks (not to include New Jersey State Police Barracks located on toll roads) to where they are geographically located.

Once your application has been submitted to the Berlin Police Department you will be provided with a “MorphoTrak” application and will be directed to contact this private vendor either by phone or internet to schedule an appointment. Morpho Trak will be responsible for taking fingerprints which will be sent to the FBI and NJ State Police for the criminal record check. There will be an additional fee which is indicated on the Morpho Trak form. Please be advised once you received the Morpho Trak Form from this office you will have thirty (30) days from the date you submit your application to get fingerprinted.

Application Instructions for REQUEST FOR CRIMINAL HISTORY RECORD INFORMATION (Form 212A)

Additional Permits to Purchase a Handgun and/or Duplicate Firearms Purchaser Cards
 
  • Log on to https://www.njportal.com/njsp/criminalrecords.

  • Click on the ON LINE FORM 212A, a highlighted block located on the lower left side of the page.

  • Originating Agency Identification Number (ORI): NJ0040500

  • Follow the prompts for demographic and payment information.

  • The applicant will be required to include their State Bureau of Identification number (SBI number) during the demographic information portion of this on line application. This number can be found on your NJ Firearms ID Card after the heading “NUMBER”. The NJ SBI number format is six numbers followed by one letter. Your request will be rejected if this number is not included.

  • Upon completion of the form, you will receive an email Confirmation & Receipt that will include a confirmation number.

  • At this time the request will be forwarded to the Berlin Police Department for approval and submission to the New Jersey State Police for processing.
